Only residents of Lake County, Florida, are eligible. Complete documentation is required.

You must provide your complete current Lake County residential or lodging address, not only a ZIP code, P.O. box, or general area. You must also provide a government-issued photo ID, current photos of every pet, and vaccination records for every pet. If your ID does not show your current address, you must also upload current proof of Lake County residency. An application missing any required answer or file cannot be submitted or reviewed. Assistance depends on current donations, inventory, and organizational capacity and is never guaranteed.

Address + identity
A complete current address and government-issued photo ID are required. Address proof is required when they do not match.

Pet verification
A current pet photo and vaccination record are required for every pet listed.

Private by design
Uploaded records are stored privately for staff review and are never displayed publicly.

Your information

Eligibility begins with location and identity. You must currently reside in Lake County, Florida. Enter your complete current residential or lodging address. The name on your ID must match the owner or authorized caregiver named on each pet’s veterinary record.

Applicant verification

Upload the front of a current government-issued photo ID. Your full name, photograph, and address must remain visible. You may cover the ID number, date of birth, and signature.
